Registration

We are based within St Andrews Community Hospital, Largo Road, St Andrews and cover the following areas: Balmullo, Blebo Craigs, Boarhills, Ceres, Dairsie, Dunino, Guardbridge, Kingsbarns, Largoward, Leuchars, Peat Inn, Strathkinness and St Andrews. Look at a map of our Practice boundary.

We are unable to register patients who live outwith the Practice boundary. We do not register patients who work in St Andrews but live outwith the Practice boundary.

Simply follow the next 4 steps:

Step 1:

The quickest and easiest way to register is by downloading and completing the below form using Acrobat Reader and returned to the Practice by email to Fife.registration@nhs.scot.  Unless your forms have been filled out incorrectly and we need to contact you for additional information, we will NOT respond to your email and you can assume your registration will be complete within five working days.  If you do not have access to email, the forms can be completed, printed off and returned to the practice in person or by post to Pipeland Medical Practice, St Andrews Community Hospital, Largo Road, KY16 8AR (please ensure you attach sufficient postage to your envelope).  If you do not have the ability to print the forms, please telephone the Practice on 01334 476840 to discuss how we can help you complete the registration process.

Step 2:

You will also need to complete a Medical Questionnaire; this will help us get to know you and your health.  The relevant form below can be downloaded and completed using Acrobat Reader and returned along with your New Patient Registration Form as above:

Step 3:

Once the registration process has been completed, you can sign up to our Online Services if you have medication on repeat.  This service is available 28 days after your registration has been completed and will allow you to order your medication (Click “Patient Login” above right) or via the Patient Access App on iOS or Android. Please only request this service if you have repeat medication.
